ICB Event Application Form

To book your table at our April 13, 2019 Transformational Wellness Show please provide your details in the form below. All applications will be reviewed and we will notify you by email if your application has been approved. Payment of $175 via PayPal, or e-transfer must be received within 15 days of application acceptance to reserve your spot at our TWS. All applications must be submitted by February 15, 2019.

Our event will be held at The Delta Guelph Hotel And Conference Centre, Royal City Ballroom, 10 AM – 5 PM.

Cost of sponsorship is $300 per event. Your logo will appear on all event advertising, including the ICB website, Facebook page (social media), event flyers, newspapers, radio etc.

Note - *The Transformational Wellness Show room is setup with participants located around the perimeter of the room and rows up the middle. Those applicants who require electricity are setup around the perimeter of the room.

Cancellation Policy:

  • A full refund will only be given where written cancellation is received within 30 days of your payment.
  • No refund will be given for events cancelled less than 30 days prior to the event date.

(Please note, only tables that have been paid for in full will be guaranteed a spot at an event.)
